1-800 CONTACTS is acknowledged as an out-of-network provider by most major vision insurance companies, such as VSP, Cole Managed Vision, EyeMed, Davis Vision, and Spectera. Insurance companies such as these will reimburse you for your purchase, provided that you have an applicable out-of-network benefit in your plan.

If you have an out-of-network contact lens benefit in your vision plan, you can simply purchase your contact lenses through 1-800 CONTACTS and then submit the itemized receipt that you will receive with your order to your insurance company for reimbursement. The reimbursement that you will receive from your insurance company is determined by the out-of-network benefit in your vision plan.

1-800 CONTACTS is accepted as an out-of-network provider by Cole Managed Vision. If you have an out-of-network contact lens benefit, and are eligible within your Cole Managed Vision plan, you will simply submit the itemized receipt that you will receive with your order along with the Cole Managed Vision Out-Of-Network Reimbursement Form to:

An itemized receipt (this comes with your order from 1-800 CONTACTS)
Out-Of-Network Reimbursement Request Form
Subscriber’s name, address, and Spectera ID#
Patient’s name and date of birth
A note requesting reimbursement

VSP

VSP will reimburse you for your purchase of contact lenses at 1-800 CONTACTS if you have an out-of-network benefit included in your plan. To receive your reimbursement, simply submit your itemized receipt from your order along with a VSP out-of-network reimbursement form to VSP. The reimbursement form can be found on VSP’s website.
